Understanding it

What Is Failure to Launch Syndrome?

Failure to launch is the name people use when a young adult can’t seem to make the move into independent life. They may be 20, 25 or older, living with their parents, not working or studying in any steady way, and stuck in a routine that goes nowhere. It isn’t a formal diagnosis, and it isn’t laziness.

Underneath it there is usually something real: anxiety that makes every next step feel impossible, depression that drains motivation, low confidence after a few failures, and very often alcohol or drugs that make the stuckness more bearable. Over time the substance use and the stuckness feed each other.

Sunview treats young adults when substance use is part of that picture. Treatment is outpatient, so the work happens in the same life they’re trying to change, not somewhere away from it.

What families notice

Failure to Launch Symptoms in Young Adults

Six signs parents describe on the phone

Living at home with no plan to leave

Months turn into years, and conversations about moving out or getting a job end in an argument or silence.

Days without structure

Sleeping late, up all night, gaming or scrolling, and no routine that leads anywhere.

Starts that don’t stick

Classes dropped, jobs that last a few weeks, plans that sound good on Sunday and are gone by Wednesday.

Drinking or using to get through it

Cannabis every day, alcohol at night, pills to sleep or to feel something.

Anxiety and avoidance

Interviews, phone calls and new people feel impossible, so they get avoided, and avoiding them makes the next one harder.

Parents doing the adulting

Rent, bills, appointments and excuses are handled by mom and dad, and everyone is exhausted.

Why it gets stuck

How Failure to Launch and Substance Use Feed Each Other

A loop, not a character flaw

The pressure to grow up brings anxiety and shame. Drinking or using takes the edge off. The next step gets avoided, the pressure grows, and the loop runs again. Treatment works on every point of the loop at once, which is why substance use and mental health are treated together at Sunview as dual diagnosis.

The failure to launch cycle
  1. 1Pressure to grow up
  2. 2Anxiety and shame
  3. 3Using to feel better
  4. 4Avoiding the next step
  5. 5More pressure and more shame

Moving Beyond the Sick Role: Sunview’s Approach to Failure to Launch

Our CEO, Frank Galimidi, has spent years working with young adults caught in this pattern. Part of what he learned is captured in an idea we call “I = Sick”: the fear of functioning as a healthy adult once the sick role is removed.

For a young adult who has been “the one with the problem” for years, getting better can be frightening, because it comes with expectations. Treatment at Sunview addresses that fear directly and helps people redefine themselves beyond the diagnosis.

Our clinical approach

“I = Sick” is the fear of functioning as a healthy adult once the sick role is removed.

Failure to Launch Programs for Young Adults: How Outpatient Treatment Helps

Structure first, then independence, practiced in real life

Structure that holds

Group starts at set times several days a week. Showing up is the first skill, and it gets practiced from day one.

A room of adults

Young adults join the same groups as everyone else, with people a few steps ahead who are honest about the road.

Practice outside the room

With In Vivo Treatment™, the job application or the hard phone call becomes the assignment, and group is where it gets processed.

Many programs that call themselves “failure to launch programs” are residential life-skills programs. Sunview is outpatient addiction and mental health treatment. Your adult child keeps living at home or in their own place and comes to PHP or IOP at set times, with a primary therapist every week and psychiatric care through Telemed Clinix when it’s needed.

Therapy draws on Internal Family Systems, CBT, DBT and inner child work, and family therapy is part of the plan when it’s clinically recommended. As things improve, people step down from PHP to IOP to OP with the same team.

For parents

What Parents Can Do When an Adult Child Won’t Launch

You can call before they’re ready

  1. 1

    Call us yourself

You can ask questions and understand the options before your adult child agrees to anything.

  2. 2

    Plan the conversation

We’ll talk through how to bring it up, what to say, and what you’re prepared to change at home.

  3. 3

    Your adult child calls

Treatment is their decision. A short pre-screening and our Clinical Director’s recommendation come next.

  4. 4

    You stay involved

With their written consent, we contact family in the first week and include you at a pace that works.

Our Clinical Director, Dana Martin, is a couple and family therapist, and Sunview’s clinical model looks at the whole family system. For more, read our guide for families and our page on rehab for young adults.
